Employee Benefits Summary
Health and Dental Benefits
Eligible employees have a choice of PPO and HMO medical and dental plans. All plans include prescription and vision benefits.
Life Insurance
All eligible employees receive $10,000 basic life insurance. Supplemental insurance may be purchased at a minimal cost.
Long Term Disability
We provide a long-term disability plan to all eligible employees in the event you become totally or partially disabled due to injury, disease, illness, pregnancy or mental disorder.
Supplemental Insurance Policies
Individual policies are available for all eligible employees for Intensive Care, Short Term Disability and Cancer.
Retirement
All eligible employees are enrolled in a defined benefit retirement plan that offer a monthly retirement benefit at age 65 and early retirement benefits at age 55 with 120 months of service. Employees are fully vested after five (5) years of service.
Group 403(b) Tax Sheltered Annuity – A Group TSA offers employees the opportunity to save for retirement through pre-tax payroll deductions.
Educational Benefits
Education benefits are available for licensed clinical staff that are required to obtain CE Hours to maintain their license. The benefit hours available are based on years of service and FTE.
Education Assistance Program – this program provides reimbursement for tuition and books to eligible employees in qualified programs. Maximum reimbursement for .9 FTE is $5,000 and .5 FTE - .8 FTE $2,500 annually.
Relocation Assistance
The hospital will provide relocation assistance to a maximum of $6,000 to out-of-state candidates selected for full-time employment in designated recruitment sensitive positions.
Employee Referral Pay
Referral pay is available to current AVH employees who refer qualified candidates who are selected for employment at AVH in designated Recruitment Sensitive positions.
