Medical Records

We are committed to patient privacy and ensure that your medical information is kept confidential in accordance with the law. We offer two ways to securely access your patient records:

My AVMC Patient Health Portal

Antelope Valley Medical Center’s secure online patient health portal allows patients to access their health information online. In addition, a patient may designate a representative whom is also allowed to access their information.

Medical Records Department

Patients may request hard copies or a compact disc (CD) of their medical records using our medical records authorization form (English authorization) (Spanish authorization). To speak to someone directly, call 661‑949‑5011.

The Authorization for Use and Disclosure of Health Information may be obtained from the Medical Records department. We can fax, mail or email it to you, or you may come in person to the Medical Records department. Identification is required. If the authorization is signed by someone other than the patient, supporting identification and/or documents may be necessary. Special restrictions apply to Psychiatric, adoption records and HIV results. Please contact the Medical Records department for specific information regarding these circumstances.

The first abstract will be provided free of charge for discharges on or after March 13, 2006. Abstract includes demographic cover sheet, Emergency Department record, Discharge Summary (if applicable), History and Physical, Consultations, Operative Report or Procedure Note and diagnostic reports (lab, x ray, EKG, etc.) Otherwise, charges are as follows:

Pages from microfilm: .50 cents per page
Photo reproduction: Actual cost
Compact Disc: $5 (only applies to electronic records)

We make every effort to respond in a timely manner to your request for release of information. Our goal is to process your request within 10 working days from our receipt of the completed authorization form.

Birth Certificates

Birth certificates are issued by the Los Angeles County Department of Public Health. AVMC provides pertinent information to the county, which is used to create the official birth certificate. You can obtain a copy from the Los Angeles County Registrar‑Recorder/County Clerk.
